Request support for children under 5 years - for early years professionals only
We are committed to ensuring every child receives the best possible start in life. Most children with SEND will have their additional needs met by their setting. They will do this with resources ordinarily available.
A setting, or other professionals can request some extra support to do this from the Best Start in Life Advisors (BSILAs). The BSILAs work in our locality teams.
Who should use this form
The form is for:
- council colleagues. Local authority staff might seek additional resources. They may want to request interventions for children in early years settings
- voluntary sector organisations. This includes charities and non-profit organisations working with children who need extra support
- early years settings such as nurseries, preschools, and childminders. This is to request help, information, or advice for children in their care
This form is not for:
Health partners.
Healthcare professionals might identify children who may benefit from additional support services. If so, a Section 23 Notification must be completed.
Parents or carers and families.

How to complete the form
1. Personal information: Provide your contact details and the organisation you represent.
2. Child’s information: Enter the child’s details. This should include name, date of birth, and early years setting.
3. Support needed: Describe the type of support required. This might be educational resources, health interventions, or family support services.
4. Reason for request: Explain why the support is needed. Include any relevant background information or assessments.
5. Additional information: Include any other details that may help us understand the child’s needs better.
